Obtaining Your Vital Records

A step-by-step guide to getting your essential documents. We're here to help you navigate the process.

How to Get a Birth Certificate (in Maryland)

Your birth certificate is a key identity document. In Maryland, these are issued by the state's Vital Statistics Administration (VSA).

Primary Office: MDH Vital Statistics Administration

This is the main office for all Maryland birth records, including those from Baltimore City.

Location: 6764-B Reisterstown Road, Baltimore, MD 21215

Choose Your Application Method

  • Online (Fastest): Use the official state partner, VitalChek. This is the most convenient option for many.
  • By Mail: Download the application form from the VSA website and mail it in.
  • In-Person: Visit the Baltimore office. It's highly recommended to check their website or call first, as an appointment may be required.

Gather Required Information

You will need to provide specific details and documents, such as:

  • Full name on the birth certificate & date/city of birth
  • Parents' full names (including mother's maiden name)
  • A valid, government-issued photo ID for yourself
  • Payment for the fee (check website for current cost)

Key Tip:

Always start at the official Maryland Department of Health website to get the most accurate forms and instructions. You can find it at health.maryland.gov/vsa.

How to Get a Social Security Card (U.S. Federal)

Your Social Security card is vital for employment and accessing services. It's issued by the federal Social Security Administration (SSA).

Choose Your Application Method

  • Online: If you are an adult U.S. citizen, you can often apply for a replacement card online through a "my Social Security" account.
  • By Mail or In-Person: Download and complete Form SS-5. You will need to mail or bring it to an SSA office with your original documents.

Provide Proof of Identity & Citizenship

You must prove who you are and that you are a U.S. citizen. You cannot use photocopies.

  • Proof of Identity: U.S. driver's license, state-issued ID, or Passport.
  • Proof of Citizenship: U.S. birth certificate or U.S. Passport.

Crucial Reminder:

Obtaining a Social Security card from the SSA is always FREE. Be very careful of websites that try to charge you a fee for this service. Always use the official government site: www.ssa.gov.
